Join a passionate and dedicated team at the Care Progression and Improvement Hub (CPIH), where you’ll play a key role in improving patient care and hospital flow across a 7-day service. As a CPIH Coordinator, based within the Network Operations Centre, you’ll be instrumental in identifying and supporting complex or long-stay patient/clients who may be at risk of extended hospital admission due to delayed or unclear discharge plans. Your work will focus on increasing visibility of these cases and actively addressing barriers to safe and timely discharges, all while fostering collaboration and communication across multidisciplinary teams.
In this dynamic and rewarding role, you’ll work closely with internal hospital teams, external service providers and community care networks to ensure every patient has a clear and coordinated pathway home or to their next care setting. You’ll bring a proactive, solution-focused mindset to each challenge, escalating issues where needed and championing an “all of organisation” approach to patient care progression.
