About the Role
As an Allied Health Assistant, you will play an important role in supporting patients and their families across CALHN’s Integrated Care Geriatric Services programs, including Geriatrics in the Home (GITH), the Multidisciplinary Community Geriatric Service (MCGS), and the Geriatric Outreach Service (GOS). Working under the guidance of the multidisciplinary team (MDT), you will provide therapy support and practical assistance with daily living activities, helping to improve the wellbeing and independence of older persons.
In this role, you will contribute directly to hospital avoidance and substitution strategies by ensuring patients receive timely, compassionate and effective care in the most appropriate setting. Your support will make a meaningful difference in the patient experience, while also helping families feel supported and connected throughout the care journey. About You
You are a compassionate and motivated Allied Health Assistant with the qualifications, skills, and experience to support patients and families in a healthcare or community setting. With excellent communication and teamwork abilities, you thrive in multidisciplinary environments while also working independently to deliver safe, effective, and person-centred care.

Hold a Certificate III in Allied Health Assistance (or equivalent qualification in aged care, community health, disability, or a related health field) with experience in healthcare or community settings.

Strong interpersonal and communication skills, with the ability to engage with patients from diverse backgrounds and work collaboratively within a multidisciplinary team.

Highly motivated, reliable, and adaptable, with the ability to manage workloads, follow care plans, use initiative, and maintain professionalism in demanding environments.

Committed to ongoing professional development, with sound knowledge of MDT practices and the ability to apply digital and administrative skills to support patient care.
