• Department for Health and Wellbeing – Clinical System Support and Improvement – Integrated Care Systems
• Salary: $118,887 to $129,427 – AHP4 – Full Time / Term Contract up to 30 June 2027

Overview:

• The Clinical Project Manager, Geriatric Outreach Service is responsible for leading and managing state-wide strategic projects and reform initiatives related to the aged care and disability programs to the community. 
• The role manages complex projects relevant to the development and implementation of the Geriatric Outreach Service with the Local Health Networks (LHN), leads and manages its implementation, initiatives and provides specialist advice to the Department for Health and Wellbeing, and SA Government and non-Government agencies, as required
• The Clinical Project Manager, Geriatric Outreach Service will significantly contribute as an integral team member of the Integrated Care Systems Directorate of Department for Health and Wellbeing.
