About the role

We currently have an exciting opportunity to join the CALHN Diabetes Education Service as a Clinical Nurse, Diabetes Education. The role is patient facing, educating patients with all types of diabetes to assist them in their daily self-management in and out of hospital.

The CN Diabetes Education:

  • Is responsible and accountable for providing nursing services and the provision of quality patient centred care to adults (with their carers) with type 1, type 2, type 3C, LADA, steroid induced, post-transplant, and cystic fibrosis related diabetes mellitus.
  • Works with other specialties include renal, cardiology, cardio thoracic surgery, surgery, haematology, oncology, respiratory, vascular, podiatry, and islet cell transplants
  • Works in partnership with the person with diabetes (and significant others) and in collaboration with other health professionals to ensure maximum health outcomes, using patient-centred model of care
  • Accepts accountability for their own practice standards and responsibility for activities delegated to others, as well as the guidance and development of less experienced staff
  • Consolidates knowledge and skills, and develops capacity through experience and continuous professional development
  • Works to actively seek, assess, implement, and evaluate quality improvement activities
  • Practices within agreed evidence-based practice standards and integrates advanced clinical knowledge; demonstrates the necessary competency to provide care relevant to each patient’s needs and agreed patient care outcomes
  • Works with and develops in the use of information technology (IT) for successful use of software programs with the patient, to assist the management of insulin pump therapy and continuous glucose monitoring

About You

We are looking for a dedicated and experienced Clinical Nurse who is passionate about delivering high-quality patient care and supporting the development of others. To be successful in this role, you will have:

  • Qualifications & Experience – Current registration or eligibility with the Nursing Board of Australia, with at least 3 years of post-registration experience.

  • Clinical Expertise – Proven competence in acute/subacute care and diabetes management, along with experience mentoring and guiding less experienced staff.

  • Skills & Abilities – Strong communication, problem-solving, and teamwork skills, with the ability to prioritise tasks and work independently.

  • Knowledge & Leadership – A solid understanding of healthcare legislation, quality improvement systems, and contemporary nursing practices.
