About the Role

The Nurse Consultant Disability/NDIS and Complex Care will provide clinical nursing expertise to patients and provide clinical leadership and in a variety of clinical settings which could include community outreach. Employees classified at this level are experts within their area and stream of practice and have a significant degree of autonomy and decision making.

This position is to maintain contemporary knowledge about hospital avoidance and discharge strategies and internal and external services to coordinate safe patient discharge, and prevent unnecessary CALHN presentations and admissions, provide clinical leadership and education within the organisation, and act as a CALHN expert in the National Disability Insurance Scheme and management of patients with complexities, inclusive of building effective working relationships with relevant Government and Non-Government Organisations and internal stakeholders.

The position is to work alongside CALHN programs and as a part of a Multidisciplinary team to support hospital demand by navigating the discharge of those with complexity, and escalating barriers whist maintaining clear documentation, data and reporting. This role will be required to both adapt and respond to changing landscapes which could include roles and responsibilities and models of service delivery.

About You

You are a registered nurse or midwife with a strong clinical background and a passion for leading high-quality care in multidisciplinary settings. Your leadership, adaptability, and commitment to innovation enable you to drive positive change and foster a supportive workplace culture.

Key requirements:

  • Qualifications and Registration: Registered or eligible for registration with the Nursing and Midwifery Board of Australia and holding (or eligible to hold) a current practising certificate.

  • Leadership and Teamwork: Proven ability to lead teams, manage change, promote consumer engagement, and develop staff potential in dynamic healthcare environments.

  • Clinical Expertise and Experience: Minimum three years post-registration experience, with advanced skills in triage, assessment, and decision-making in complex community settings.

  • Knowledge and Compliance: Strong understanding of relevant legislation, standards, and contemporary nursing/midwifery issues, with a focus on safety and quality in care delivery.
