About The Role

As a Community Health Nurse, you will provide high-quality, person-centred nursing care to clients across community-based settings within Country Health Connect. Working with increasing autonomy, you will assess individual needs, plan and coordinate care, deliver nursing interventions and connect clients with other health professionals and services to support positive health outcomes.

You will work collaboratively within a multidisciplinary team while maintaining accountability for your own clinical practice and contributing to client safety, health promotion and quality improvement.

About You

You are a compassionate and capable Registered Nurse with strong clinical knowledge, sound assessment and decision-making skills, and a commitment to providing person-centred care. You are confident working autonomously when required while collaborating effectively with multidisciplinary teams to support the diverse and sometimes complex needs of clients in the community.

To succeed in the Community Health Nurse role, you will:

• Be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and hold, or be eligible to hold, a current practising certificate.
• Demonstrate competence in relevant nursing practice and a commitment to delivering safe, evidence-based, consumer and family-centred care.
• Bring effective communication, critical thinking, problem-solving, conflict resolution and negotiation skills, with the ability to work effectively within a multidisciplinary team.
• Demonstrate the ability to assess and prioritise client needs, manage your workload and work autonomously when required in community-based settings.
• Demonstrate an understanding of culturally appropriate care, including the ability to engage respectfully with Aboriginal consumers and communities and a willingness to further develop your cultural knowledge and capability.

Qualifications relevant to Community Health Nursing, Wound Care, Primary Health Care, Health Promotion, Public Health, Chronic Disease Self-Management or Immunisations, along with previous experience in a community health setting, are desirable.

Country Health Connect provides community-based health services that support people to maintain their health, wellbeing and independence within their own homes and communities.

As part of the Community Nursing team, you will work with clients across different ages, backgrounds and levels of complexity, providing safe clinical services in community and home-based environments. You will collaborate with other health professionals, disciplines and external agencies to assess individual needs, coordinate appropriate services and support continuity of care.

The role also provides opportunities to contribute to health promotion and education, quality improvement, evidence-based practice and the development of less experienced staff and students.

Conditions of Employment

• A current Nationally Coordinated Criminal History Check (NCCHC) for employment involving unsupervised contact with vulnerable groups is required for this position.
• A current Working With Children Check (WWCC) issued by the Department of Human Services is required.
• A current NDIS Worker Screening Check is required.
• A current South Australian Driver’s Licence is required.
• The successful applicant may be required to work at other Country Health Connect sites across the Limestone Coast and may be required to participate in an on-call roster.
• Immunisation Risk for this position is – Category A 
• SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy in the workplace.
