About the Role:

Join our team as a Care Partner and make a meaningful difference in the lives of older people by supporting them to live safely, independently and with choice in their own homes. Working collaboratively with clients, families and carers, you’ll use your expertise and professional judgement to develop strengths-based, goal-directed care plans and create flexible, personalised solutions that reflect each client’s unique needs, goals and preferences.

What We're Looking For:

Essential Criteria 

  • Provide person-centred case management, supporting clients to maintain independence, wellbeing, dignity and choice at home.
  • Develop, monitor and review care plans and budgets, responding flexibly to changing client needs and complex circumstances.
  • Maintain accurate documentation and administration, including client records, reporting, equipment management and compliance with policies and procedures.
  • Work collaboratively with clients, families, carers and multidisciplinary teams to ensure coordinated, respectful and culturally sensitive care.
  • Contribute to quality improvement and professional development, including home safety, health promotion, staff development and maintaining high standards of service delivery.

Desired Criteria 

  • Strong communication, interpersonal and teamwork skills, with empathy and the ability to work effectively with clients, families, colleagues and community agencies.
  • Relevant aged care and community experience, including client-directed care, social inclusion and supporting people with complex needs.
  • Ability to work independently and prioritise effectively, demonstrating sound judgement, flexibility, problem-solving skills and a commitment to person-centred care and continuous improvement.
