Beyond the Bedside: Clinical Nurse – Hospital In The Home

This role delivers hospital-level care where it matters- in the Home and Communities of Adelaide’s South.  Flinders Medical Centre is seeking committed Clinical Nurses to join Hospital In The Home (HITH) program within the Southern Adelaide Local Health Network (SALHN), bringing expert clinical care directly to patients, who can have treatment safely outside of a hospital ward.

About the role: 

As a clinical nurse within the HITH team, you will manage and independent patient case load across the southern Adelaide Metropolitan area, coordinate and delivering acute-level care in the community and home settings, reporting to the Nurse Unit Manager of the HITH service.

Your key responsibilities will include:

  • Providing evidence-based nursing care including experience with managing PICC, IVC and ports, IVC cannulation, administration of medications,
  • Acute wound management skills,
  • Completing comprehensive clinical assessment and escalating care in line with SALHN protocols,
  • Collaborating with the multidisciplinary team to plan, review and transition patient care,
  • Supporting patients and families with education to promote safe self-management and recover,
  • Working over a 7-day roster

About you:

  • Current registration as a Registered nurse with the Nursing and Midwifery Board of Australia AHPRA,
  • Demonstrated experience in acute medical, surgical or community nursing with at least s years post graduate experience,
  • Confidence working autonomously in unstructured and variable care environment,
  • Sound clinical decision-making and prioritisation skill across a mixed patient caseload,
  • Current unrestricted South Australian Drinver Licence,
  • Familiarity with SA Health clinical standards and electronic medical record systems,
  • At least 3 years post registration

In this role you will have strong interpersonal and communication skill which will enable you to successfully problem solve, resolve conflicts and negotiate, including the ability to engage in difficult conversations, whilst building and maintaining relationships. An ability to effectively manage multiple, diverse and competing priorities and demonstrated resilience with a stressful and complex environment is essential to this role
