About the role
Shape the future of Integrated Care across CALHN, leading safe, connected services and a high performing medical workforce. Partner across the system to improve flow, strengthen models of care and deliver better outcomes for patients and communities.

• Lead the IC medical workforce and clinical services
• Shape safe, connected models of care and service reform
• Drive clinical governance, education and research
• Build strong partnerships across hospital and community care
• Advise CALHN leaders on medical workforce and clinical priorities

About Integrated Care

Integrated Care brings together innovative, interdisciplinary services across hospital and community settings to deliver the right care at the right time. Spanning five locations, the program supports hospital avoidance, faster access to care and smoother transitions between primary, acute and tertiary services. Working closely with partners across the health system, Integrated Care delivers services including Hospital in the Home, Urgent Care Hubs, rapid assessment, virtual care, aged care assessment, Refugee Health and GP integration.

About You
We’re looking for a collaborative medical leader who can turn strategy into better care, stronger teams and sustainable service improvement.

• Hold specialist qualifications and AHPRA registration
• Lead clinical teams and cross functional collaboration
• Drive service reform across hospital and community settings
• Apply strong clinical, financial and workforce governance
• Build partnerships that improve consumer outcomes
