About the Role

As a Community Health Nurse, you'll provide person-centred nursing care across a variety of community and home-based settings, supporting individuals and families to achieve the best possible health outcomes. Working both independently and as part of a multidisciplinary team, you'll assess client needs, coordinate care, deliver health education, and contribute to health promotion initiatives. This rewarding role offers the opportunity to build strong relationships within the community while delivering high-quality, evidence-based care across the Limestone Coast region.

About You.

You are a compassionate and adaptable Registered Nurse or Midwife who enjoys working autonomously while collaborating with a multidisciplinary team. You are committed to delivering high-quality, person-centred care and building positive relationships with clients, families and community partners.

To succeed in this role, you will have:

  • Registration (or eligibility for registration) with the Nursing and Midwifery Board of Australia and a current practising certificate.
  • Demonstrated clinical nursing experience with the ability to provide safe, evidence-based care.
  • Excellent communication, critical thinking and problem-solving skills.
  • The ability to work independently while effectively managing competing priorities.
  • Experience in community nursing (desirable) and a commitment to continuous professional development.
