Join our SALHN Physiotherapy Team

Lead, design and implement Physiotherapy services within the GEM@Home Service to meet the needs GEM@Home Model of Care and the patient cohort.

About the role: 

Reporting to the Advanced Nurse Unit Manager - GEM @Home, this role is integral to providing care to older adults with complex medical, functional and/or psychosocial needs within their own homes in lieu of an extended or avoidable hospital admission.

Duties for the role will include:

  • Providing leadership, consultancy and advice to management and other health professionals on service delivery development and practice within Physiotherapy services in GEM@Home and the broader GEM services as required.
  • Monitoring the prioritisation, efficiency and effectiveness of Physiotherapy services to patients within GEM@Home
  • Supervising junior physiotherapy staff, and Allied Health Assistants within the GEM@Home service
  • Assessing patients’ physical abilities, problems and needs using a variety of formal and informal assessments suited to this specific patient group
  • Documenting assessment findings and interventions, in accordance with documentation guidelines and standards
  • Communicating with client’s family, carers and other service providers, including linking clients and carers with relevant community resources and services
  • Actively participating in continuous Quality Improvement activities, including the identification of performance standards and increased efficiencies.
  • Participating in the recruitment and selection of staff as required.
  • Undertaking clinical training and education activities, including the development of training resources.

About you:

  • A degree or equivalent which entitles registration as a Physiotherapist with the Physiotherapy Board of Australia
  • Extensive experience in the provision of clinical Physiotherapy services to clients with a broad range of illnesses and disabilities, in particular clients with complex needs including those with cognitive impairment and/or a diagnosis of dementia.
  • Demonstrated ability to assume responsibility for the organisation and supervision of clinical Physiotherapy services to a designated area as a member of an interdisciplinary team. This includes the ability to undertake assessment and intervention with clients with a variety of illnesses and disabilities, including management of complex cases.
  • Demonstrated high level interpersonal skills including negotiation, consultation and conflict resolution skills, and the ability to establish and maintain effective networks with key stakeholders.
  • Demonstrated high level communication skills including the ability to communicate with clients, professional staff of different disciplines, administrative staff and staff of other agencies, both verbally and in writing.
  • Demonstrated experience in clinical management and leadership within a discrete unit, including program evaluation and service development.
  • Demonstrated experience in the effective development, implementation and evaluation of quality improvement initiatives, and / or departmental policies and procedures relevant in a community setting
