Join SA Health as a Senior Social Worker - Rehabilitation in the Home (RITH) at Modbury Hospital and make a meaningful difference in the lives of patients and their families across Adelaide's northern suburbs. As part of the Northern Adelaide Local Health Network, you will provide expert face to face and tele rehabilitation social work services to patients requiring in home rehabilitation, helping facilitate safe and timely transitions from hospital to community care.
Key Responsibilities
- Conduct comprehensive psychosocial assessments for patients and families
- Develop and implement discharge plans in collaboration with multidisciplinary teams
- Provide crisis intervention and brief counselling services
- Coordinate community resources and support services
- Mentor junior staff and contribute to team development
To be eligible for this position, you must have appropriate Degree or equivalent qualification which entitles membership as an Social Worker with the Australian Association of Social Workers.)

In this role you will have strong interpersonal and communication skills which will enable you to successfully problem solve, resolve conflicts and negotiate, including the ability to engage in difficult conversations, whilst building and maintaining relationships. An ability to effectively manage multiple, diverse and competing priorities and demonstrated resilience within a stressful and complex environment is essential to this role.
