About the Role

Senior Discharge Coordinator drives complex discharge and NDIS pathways, ensuring safe, timely care.

• Responsible for expert clinical leadership and an expert consultative service to support the MDT
• Manage complex cases.
• Lead complex discharge plan across NDIS pathways
• Coordinate MDT care for high needs patients
• Lead and support Service/Quality Improvement activities 

About You

You bring strong allied health expertise and a passion for complex care and safe discharge outcomes.

• Relevant degree eligible for AHPRA or AASW registration
• Strong clinical leader with collaborative MDT approach
• Expert complex discharge planning and NDIS pathway knowledge
• Highly organised, strong communicator and problem solver
• Experience in supervision, education and service improvement
