About the role: 

  • Provide compassionate relationship centred nursing care and/or individual case management to consumers that involves carers and building knowledge of the consumer and carer as individuals and as a network of relationships which provide human connection.
  • Participate in quality improvement activities that contribute to consumer’s safety, risk minimisation and safe work activities within the practice setting.
  • Required to, within pre-determined guidelines, and in a multi-disciplinary primary health care setting, assess clients, select, and implement different therapeutic interventions, and/or support programs and evaluate progress
  • Participate in clinical teaching, overseeing learning experience, and goal setting for students, new staff and staff with less experience.

About you:

  • Demonstrated competence caring for residents living with dementia who experience associated behavioural and psychological symptoms at a very severe to extreme level.
  • Demonstrated competence in Mental Health nursing practice in accordance with the relevant standards.
  • Ability to take on different perspectives and work effectively within a multidisciplinary team.
  • Ability for self-reflection and to take on constructive feedback.
