Make a Meaningful Difference: Care Navigator

The SAVAD-CNS provides care coordination, education and support for South Australian people accessing voluntary assisted dying (VAD) as they live with a life limiting illness. The SAVAD-CNS also assists medical practitioners, healthcare teams and private and public health services across South Australia to navigate the VAD process. 

Care Navigators act as a role model and consultant to other health service providers (in both the South Australian metropolitan and regional areas), providing knowledge and clinical expertise in the area of VAD.

About you:

  • Demonstrated competence in the delivery of relevant models and practices to clients with a broad range of illnesses and disabilities, in particular clients with complex and end of life care needs
  • A demonstrated ability to introduce new ideas/concepts and to influence change
  • Registered Nurse and or Midwife with at least 3 years post registration experience
  • Demonstrated well developed interpersonal skills that foster trust, respect, confidence and cooperation with colleagues, patients, families, multi-disciplinary team members and senior staff

This is a single vacancy, with two possible appointment classifications. The successful applicants will be appointed in the position appropriate to their skills, qualifications, and experience.

Please note that this position is multi-classed as either a AHP3, RNM3 and classification will be determined by qualifications and experience.
