About the Role

Limestone Coast Local Health Network is seeking a motivated and experienced Community Care Coordinator to join Country Health Connect. This role supports the delivery of community-based ageing, disability and home support programs under a Consumer Directed Care (CDC) model, empowering clients to remain living independently in their own homes.

As part of a multidisciplinary team, you will coordinate, implement and evaluate programs that respond to client choice and goals, while ensuring services are compliant, financially sustainable and aligned with Commonwealth and State funding requirements.

This is a dynamic role in a changing sector, ideal for someone who thrives in complex environments and is passionate about supporting frail aged and people living with disability in the community.

Key Responsibilities

  • Coordinate and oversee community-based ageing and disability programs, ensuring compliance with funding, quality and reporting requirements
  • Lead and supervise staff and volunteers, including rostering, performance support and workforce planning
  • Conduct and support comprehensive client assessments and care planning in line with Consumer Directed Care and wellness/reablement principles
  • Monitor budgets, data collection, service delivery standards and continuous quality improvement processes

What You’ll Bring

  • Demonstrated experience in aged care, disability, community health or human services, including supervisory or leadership experience
  • Strong understanding of the Consumer Directed Care model, Home Care Packages, Commonwealth Home Support Program and/or NDIS
  • Excellent communication, negotiation and stakeholder engagement skills across multidisciplinary teams and external agencies
  • High-level organisational, analytical and problem-solving skills, with the ability to work autonomously and manage complex client needs
