Support Older People to Live Independently in Their Own Homes

Make a meaningful difference by delivering compassionate, person-centred care that empowers older people to maintain independence, wellbeing and quality of life within their chosen environment.

About the Role:

The Community Health Paramedical Aide provides high-quality, home-based care to clients under the Commonwealth Home Support Programme (CHSP). Working within a wellness and reablement framework, you will support frail older people aged 65 years and over (50 years and over for Aboriginal and Torres Strait Islander people) to remain living safely and independently at home and in the community.

You will work collaboratively with clients, families, carers and advocates to develop and implement individualised care plans. Using your professional judgement, you will deliver flexible, responsive and culturally appropriate services aligned with CHSP guidelines, ensuring client choice, dignity and high-quality outcomes are always at the centre of care.

What We’re Looking For:

  • Experience and knowledge in health/aged care and how that relates in the community setting.
  • Experience in identifying needs and working closely with clients, families and carers in a multidisciplinary team.
  • Proven experience in exercising own judgement and initiative in the day-to-day execution of the position.
  • Experience in the use of computer programs e.g. Microsoft Word, Excel and CME.
