ABOUT THE ROLE

An exciting opportunity is now available to be part of new service development within the Integrated Primary Health Care Services program, proudly funded by Country SA Primary Health Network.

Yorke & Northern Local Health Network is seeking an experienced Clinical Nurse to deliver advanced cardiac and respiratory nursing services within a community-based, primary health care model. This role focuses on chronic disease management, prevention, education and care coordination, supporting people to better manage their health and remain well in the community.

Working as part of the Patient Journey Team, you will collaborate closely with GPs, multidisciplinary clinicians and external partners using a co-design approach to improve access, continuity of care and health outcomes for individuals living with cardiac and respiratory conditions.

HOW WILL YOU CONTRIBUTE?

  • Deliver advanced, person-centred cardiac and respiratory nursing care and chronic disease management services in community health settings.
  • Plan, implement and evaluate prevention, education and self-management strategies aligned with primary health care principles.
  • Coordinate and integrate care across multidisciplinary teams, general practice, hospitals and non-government organisations.
  • Provide clinical leadership, education and support to less experienced staff, students and colleagues, contributing to service development and quality improvement initiatives.

WHO ARE WE LOOKING FOR?

  • A Registered Nurse eligible for registration with the Nursing and Midwifery Board of Australia, with a minimum of three years’ post-registration experience.
  • Demonstrated clinical expertise in cardiac and/or respiratory nursing, chronic disease management or community health settings.
  • A confident, adaptable clinician with strong communication skills and the ability to work autonomously while collaborating effectively within a multidisciplinary team.
  • A motivated professional with a passion for innovation, service development and improving access to care for regional and rural communities.
