Are you a passionate Social Worker seeking variety, autonomy and the opportunity to truly make a difference?

We are looking for a Rural Generalist Social Worker (AHP2) to join our Country Health Connect team in the Limestone Coast. This role offers a diverse and rewarding caseload across inpatient, subacute and community settings, supporting individuals and families across the lifespan.

Working with reduced clinical direction, you will apply your growing generalist and/or specialist expertise to deliver comprehensive, integrated and person-centred care tailored to rural communities.

About the Role

As a Rural Generalist Social Worker, you will:

  • Deliver Social Work services across acute, subacute and community programs
  • Provide counselling, psychosocial support and care coordination
  • Work collaboratively within a multidisciplinary team
  • Contribute to service development and quality improvement
  • Support patients and families through complex health journeys

Depending on service needs, you may rotate across areas including medical and surgical wards, rehabilitation, palliative care, paediatrics, Commonwealth Home Support Program (CHSP), NDIS and regional outreach clinics.

Key responsibilities:

Clinical Practice

  • Undertake comprehensive psychosocial assessments and interventions
  • Manage a complex caseload across hospital and community settings
  • Provide counselling to support adjustment, grief and loss
  • Support safe discharge planning and transition of care
  • Deliver outreach and home-based services, including use of telehealth technologies

Multidisciplinary Collaboration

  • Contribute Social Work expertise to integrated care planning
  • Work closely with medical, nursing and allied health colleagues
  • Link patients and carers with relevant community and government supports
  • Participate in team meetings, case conferences and regional networks

Professional Development & Supervision

  • Work under formal clinical supervision arrangements
  • Contribute to supervision of junior staff, allied health assistants and students (as delegated)
  • Engage in ongoing professional development and reflective practice
  • Participate in the Regional Social Work Network

Service Development & Quality Improvement

  • Contribute to service evaluation and quality improvement initiatives
  • Assist in developing clinical resources and protocols
  • Monitor outcomes and ensure timely, accurate documentation
  • Support innovation in rural service delivery models

Who Are We Looking For?

You will bring:

  • A recognised qualification in Social Work
  • Eligibility for practising membership with the Australian Association of Social Workers (AASW)
  • Demonstrated clinical experience in health, rehabilitation, community or subacute settings
  • Ability to manage a varied and complex caseload
  • Strong communication, problem-solving and organisational skills
  • Experience working within multidisciplinary teams
  • A commitment to culturally safe practice, particularly with Aboriginal communities
  • Flexibility and willingness to travel across the region

A current driver’s licence is essential.

Experience working in rural or remote settings is highly regarded.

ABOUT YOU

  • You will bring experience in primary and acute health care settings, Social Work assessments, counselling, and working in a multi-disciplinary team.
  • Must hold a recognised qualification within the Social Work profession, and be eligible for practicing membership of the Australian Association of Social Workers. It is desirable to participate in the Australian Association of Social Workers accredited continuing professional development program as a requirement of a self-regulated profession.
