About the Role

Eyre and Far North Local Health Network (EFNLHN) is seeking a dedicated Case Coordinator to join Country Health Connect within Community & Allied Health. This role is based in Wudinna and Elliston and plays a vital part in supporting older people to remain living independently at home by delivering comprehensive care coordination under Commonwealth funded home support programs, including Support at Home and the Commonwealth Home Support Program.

You will work closely with clients, carers, service providers and multidisciplinary teams to assess needs, coordinate services and review outcomes, ensuring care is person-centred, culturally appropriate and aligned with the Aged Care Act 2024. Key Duties

  • Coordinate and review care plans that support clients to live independently
  • Assess client needs and connect them with appropriate health and community services
  • Advocate for clients while promoting self-advocacy and informed decision-making
  • Maintain accurate records, data and reporting to support service delivery and improvement
