About the role

RN2: As an Aged Care Needs Assessor- Clinical with Statewide ACAS, you will make a meaningful difference by undertaking complex, holistic clinical assessments for older people across community and hospital settings. Using the Integrated Assessment Tool (IAT) within the Aged Care Assessor application and My Aged Care systems, you will complete comprehensive, needs-based assessments, accurately record outcomes, and support informed, person-centred decisions that help clients access the right care at the right time.

In this collaborative role, you will guide clients and carers through appropriate aged care pathways, including in-home support, residential care, respite and flexible options funded under the Aged Care Act 2024. You will work closely with health professionals, service providers and key stakeholders, while ensuring compliance with the My Aged Care Assessment Manual, Commonwealth benchmarks and KPIs. Alongside assessment work, you will contribute to education, quality assurance and continuous improvement activities, supporting high-quality, accountable aged care services. 

AHP2:  As an Aged Care Needs Assessor- Clinical with the Statewide ACAS, you will play a vital role in supporting older people to navigate their care options with confidence. Working across community and hospital settings, you will undertake comprehensive, person-centred assessments using the Integrated Assessment Tool (IAT) within my Assessor application and the My Aged Care client record, ensuring accurate, timely and high-quality assessment outcomes.

In this role, you’ll be a trusted guide for clients and their families, providing clear and up-to-date information on the aged care pathways available – from in-home support to residential, respite, or flexible care options. You’ll also collaborate with carers, health professionals, service providers and key stakeholders to create a smooth and supportive experience.
