About the Role

Step into a role where you support patients and families to navigate change, build resilience and improve wellbeing across our CALHN sites.

• Deliver social work consults and counsel patients and families
• Guide and support junior social workers and students
• Drive quality improvement and service planning
• Work closely with a multi-disciplinary team
• Link patients with referrers, community services and follow-up care

About You

You’ll thrive in this role if you bring strong clinical capability, compassion and a commitment to high-quality, patient-centred care.

• Hold a social work degree eligible for AASW membership
• Deliver skilled assessment, therapy and evidence-based care
• Work confidently within a multidisciplinary team
• Communicate clearly with clients, carers and colleagues
• Manage priorities and maintain accurate, timely records
