Southern Adelaide Local Health Network is seeking a passionate, talented, and caring Social Worker to join the Mental Health Services Team!

The Mental Health Hospital in the Home Service (HITH) is an alternative option to hospital admission for the person experiencing an acute episode of mental illness.

The aim of HITH is to provide a hospital level of care that is high quality evidenced based interventions, treatments, and therapies in a person’s own home; enabling them to feel valued, empowered and connected to family or carers.

About the role: 

As the Social Worker duties for the role will include:

  • Developing, implementing and evaluating specific social work therapeutic interventions eg individual counselling, family interventions, crisis intervention, brief therapy and focused psychological strategies
  • Liaising with and requesting the assistance of other disciplines when necessary to contribute to the care and recovery of the consumer
  • Evaluating the progress made with each consumer, and when appropriate, facilitating transfer of care in a timely manner
  • Linking and co-ordinating an appropriate range of resources and specialist and general services within and across sectors

About you:

  • Ability to provide core social work skills including psychosocial assessment, counselling, individual and group work, relationship and family interventions, crisis intervention and brief therapy
  • Ability to undertake comprehensive assessment skills including, consumer needs analysis, assessment skills in mental state and risk assessment
  • Ability to work collaboratively and cohesively with consumers, carers, other agencies, community services and the multi-disciplinary team
  • An appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers
