About the Roles:

The Northern Adelaide Local Health Network is seeking Clinical Nurses to join our Aged Care, Rehabilitation and Palliative Care division within our community teams.

We currently have vacancies within our Northern Reablement and Intermediate Care to return Home (NRICH) and Transitional Care Program (TCP).

Clinical Nurse - Northern Reablement and Intermediate Care to return Home (NRICH) - 32 hours per week, 7 day roster.

NRICH: our dynamic multidisciplinary team, shares a transformative approach to healthcare delivery by helping patients regain independence and confidence as they transition from hospital to home. The Northern Reablement and Intermediate Care to Return Home (NRICH) Service is delivered in partnership with the Aged Care and Housing (ACH) Group, at the HEALTHIA Residential Aged Care Facility, Elizabeth Vale. ACH Group provides specialist aged care support for NRICH patients across 24 hours, 7 days per week. The NALHN Geriatric Service multidisciplinary team provides medical oversite, care planning and a reablement program to optimise functional independence in a home-based setting to support safe discharge to home.

Clinical Nurse - Transition Care Program (TCP) - 38 hours per week, 5 day roster.

TCP: is an established Commonwealth funded specialist subacute program. The program delivers short term care that seeks to optimise the function and independence of older people following a hospital stay. Transition care is goal orientated, time limited and therapy focused. The program provides older people with packages of services that include low intensity therapy such as physiotherapy, occupational therapy as well as social work, nursing support or personal care to maintain and improve physical and or cognitive functioning. The NALHN TCP includes both residential (within a Residential Aged Care Facility (RACF)) and community tranches to support exit from hospital and to avoid premature entry into residential care. Providing high-quality transitional care services to patients transitioning from hospital to community-based settings.

To be eligible for this position, you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
