About the Role:

Riverland Mallee Coorong Local Health Network (RMCLHN) has adopted a Consumer Directed Care model for aged care, disability, and home support services. This approach gives consumers greater control over their lives by enabling them to make choices about the services they receive and who provides them.

As a Case Manager, Home Care Packages, you will provide high-quality, home-based care that helps consumers remain living in their chosen environment. You will work with consumers, their families, carers, and advocates to identify personal goals, develop Home Care Agreements and Care Plans, and ensure services are tailored to meet individual needs.

With guidance from the Aged Care Coordinator – Packaged Care, you will use your professional expertise and judgement to:

  • Deliver flexible and appropriate care solutions within budget and risk parameters.
  • Support consumers to set and achieve their goals.
  • Promote independence, choice, and wellbeing.
  • Apply a strengths-based, goal-directed approach to service design and delivery.

What We’re Looking For:

ENC/END

  • Enrolled or eligible for enrolment as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
  • Experience in the provision of nursing care in the healthcare setting in accordance with the appropriate standards of practice.

OPS2

  • Demonstrated experience working in the aged care sector and providing social support programs.

  • DHS National Disability Insurance Scheme (NDIS) Worker Check 
  • National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups required for this position (E.g. aged care employment or work involving vulnerable clients)
  • Immunisation Risk for this position is - Category A
