About the Role:

Step into a dynamic leadership role as the Associate Nurse Unit Manager – Out of Hospital Strategy, where you’ll provide expert nursing and/or midwifery services across diverse health settings. Building on your experience and advanced skills, you’ll progress from competent to proficient practice, taking accountability for your own high standards, guiding less experienced staff, and supporting the Nurse/Midwife Unit Manager in leading a committed nursing and midwifery team.

In this pivotal role, you’ll:

  • Promote continuity and consistency of care in collaboration with senior nursing leaders.
  • Drive practice improvements and support change implementation.
  • Contribute to ward, unit, and service management, including staffing, rostering, performance management, recruitment, and resource planning.

As the Out of Hospital Liaison Nurse, you’ll consult with A&E and acute care teams to facilitate early discharges, prevent unnecessary admissions, and ensure seamless community-based care. You’ll also lead the development and delivery of the Better Care in the Community initiatives, including Country Home Link (CHL) Coordination and Rapid Intensive Brokerage Services (RIBS), with the goal of:

  • Reducing emergency department presentations and hospital admissions.
  • Shortening inpatient length of stay.
  • Supporting timely transfers from metropolitan services back to local communities.
  • Improving service access for vulnerable groups.

About You:

  • Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
  • Registered Nurse/Midwife with at least 3 years post registration experience or currently classified as a Clinical Nurse/Midwife.
  • Demonstrated competence in the relevant area of nursing practice in accordance with the appropriate standards of practice.
  • Experience in the supervision of student nurses, enrolled nurses and less experienced registered nurses.
