Overview

Seeking an experienced Nurse who can manage discharge planning activities for the Gumeracha District Soldiers’ Memorial Hospital. Away from the hustle and bustle of a busy metropolitan city, you will enjoy a good work life balance, flexible family friendly rostering and a supportive workplace culture situated within the stunning Barossa Valley/Adelaide Hills region. 

The Associate Nurse Unit Manager – Discharge Coordinator assists with and provides professional high quality patient care within the Gumeracha District Soldiers’ Memorial Hospital aimed at improved health outcomes for patients. Providing direct nursing care and/or individually case managing patients you will be assessing individual patient needs, planning and implementing or coordinating appropriate service delivery from a range of accepted options. You will also have demonstrated competence in nursing practice in accordance with the appropriate standards.  Requisite to this role you will have effective communication skills, conflict resolution and negotiation skills that enable you to work effectively within a multidisciplinary team.

Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia, and hold or be eligible to hold a current practicing certificate. Prioritising workloads you will be approachable, creative and flexible when managing issues within a healthcare setting.
