About The Role

Join Country Health Connect as a Community Support Worker and make a meaningful difference by supporting older people and people living with disability to maintain their independence and quality of life across the Limestone Coast.

Working directly with consumers in their homes and communities, you will provide individualised support in accordance with their care plans. This may include personal care, household assistance, meal preparation, transport, shopping and social support.

Working autonomously in the community, you will build positive relationships with consumers and families while collaborating with health professionals to provide safe, responsive and person-centred support.

About You

You are a compassionate, reliable and flexible person who enjoys supporting people to maintain their independence. You can work confidently and autonomously in the community, build respectful relationships with people from diverse backgrounds and provide practical, person-centred support in accordance with individual care plans.

To succeed in the Community Support Worker role, you will:

• Demonstrate the ability to provide personal care and practical support, which may include showering, bathing, toileting, personal hygiene, meal preparation, cleaning and laundry.
• Work effectively with consumers, families, carers, health professionals and other service providers, demonstrating strong communication, reliability and confidentiality.
• Understand consumer-directed care and reablement principles, supporting consumers to participate in decisions and maintain their independence wherever possible.
• Recognise and promptly report changes in a consumer’s health or functional status and safely undertake duties in accordance with individual care and service plans.
• Hold a current driver’s licence and have access to a reliable, registered and appropriately insured motor vehicle. A current First Aid Certificate, Certificate III in Individual Support (Aged Care, Disability or Home and Community) and/or Certificate IV in Disability are desirable.

Country Health Connect provides a broad range of aged care, disability and community-based support services across the Limestone Coast, helping people to remain independent and connected within their own homes and communities.

Working within a Consumer Directed Care model, services are tailored to each consumer’s individual needs, choices and goals. Community Support Workers play an important role in delivering this support, which may include personal care, household assistance, meal preparation, transport, shopping, social support and assistance with mobility.

You will work closely with consumers, their families and health professionals to provide responsive and respectful support that promotes independence, choice and quality of life.

Conditions of Employment

• A current Nationally Coordinated Criminal History Check (NCCHC) for employment involving unsupervised contact with vulnerable groups is required for this position.
• A satisfactory NDIS Worker Screening Check is required.
• A current driver’s licence and willingness to drive is required.
• You must have access to a reliable, registered and insured motor vehicle.
• You must be flexible and willing to participate in a 7-day roster, working varied hours/shifts across different settings.
• Immunisation Risk for this position is – Category A.
• SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy in the workplace.
