About The Role

Join Country Health Connect as a Home Support Worker and make a meaningful difference by supporting older people, people living with disability and other consumers to maintain their independence and continue living safely in their own homes.

Working within a Consumer Directed Care model, you will provide practical, person-centred support based on each consumer's individual care plan. Your duties may include general household cleaning, laundry, bed making and meal preparation.

Working autonomously in the community, you will build positive relationships with consumers and their families while collaborating with Country Health Connect health professionals to deliver reliable, respectful and high-quality support.

About You

You are a reliable, flexible and empathetic person who enjoys supporting others and takes pride in providing a high standard of service. You can work independently in community settings while also contributing positively to a multidisciplinary team, maintaining confidentiality and adapting your approach to the individual needs of consumers.

To succeed in the Home Support Worker role, you will:

  • Demonstrate the ability to work effectively both independently and as part of a team, including in a range of community settings.
  • Bring a flexible approach to work and rostering, with the ability to meet required deadlines and timeframes.
  • Communicate effectively and respectfully with people from diverse cultures, backgrounds and circumstances while maintaining strict confidentiality.
  • Have an understanding of safe working practices, infection control, cleaning and sanitising processes, customer service principles and Work Health and Safety requirements.
  • Bring some experience in housekeeping or similar duties; a current First Aid Certificate and demonstrated manual handling skills are desirable. There are no mandatory educational or vocational qualifications specified for this role.

Conditions of Employment

  • A current Nationally Coordinated Criminal History Check (NCCHC) for employment involving unsupervised contact with vulnerable groups is required for this position.
  • A satisfactory NDIS Worker Screening Check is required.
  • Immunisation Risk for this position is – Category B
  • A current driver’s licence and willingness to drive is required.
  • SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy in the workplace.
