Join our Rehabilitation, Aged & Palliative Care Team

As our Weekend Social Worker (Saturdays), you'll work closely with the Clinical Manager to help deliver seamless transition services across our network. You'll coordinate patient care, conduct meaningful social work assessments, and provide targeted interventions that support patients and families during important healthcare transitions.

About the role: 

  • Commitment to delivering high quality and safe care consistent with the SALHN Integrated Governance Framework and Consumer Engagement Framework and Plan.
  • Working cooperatively within a multi-disciplinary team to continually improve, implement and evaluate a holistic Transition Services which is responsive to identified community needs
  • Utilising specialised therapy skills to provide Social Work intervention and case management to inpatients of Transition Services wards
  • Developing and documenting patient-focussed SMART goals in active collaboration with patient, family and /or carers

About you:

  • Sound skills in building rapport and communicating with a wide range older adults, their carers /family, referrers, hospital staff and service providers
  • Eligibility for membership of Australian Association of Social Workers (AASW)
  • Strong clinical Social Work experience in working with older people and restorative programs in subacute and/or community settings
  • Ability to contribute to plans, develop, implement and evaluate community oriented, holistic therapeutic interventions appropriate for older people and their families
  • Demonstrated skills in Social Work assessment and intervention, in particular related to restorative care in older people
