About the Role

Make a real difference for older people as they transition from hospital to home or rehab. As a Transition Care Program Coordinator at RN2 or AHP2 level, you’ll manage referrals, build strong partnerships and deliver short-term care plans that support safe, supported community living.

• Coordinate intake, planning and acceptance to the Transition Care Program
• Liaise with hospitals to identify suitable patients for discharge pathways
• Develop short-term care plans and establish community case management
• Facilitate aged care assessments where required at RN2 level
• Guide practice standards and support developing nursing or allied health staff

About You

You’re a confident clinician who enjoys coordinating care, working across teams and supporting older people to transition safely beyond hospital. You bring sound judgement, strong communication and a collaborative mindset.

RN2 
• Registered Nurse with AHPRA registration or eligibility
• Clinical experience supporting older people in acute settings
• Skilled in complex discharge planning and care coordination
• Confident working within multidisciplinary teams
• Experience guiding students and developing nursing staff

AHP2 
• Allied Health degree with board registration or eligibility
• Experience in complex case management and discharge planning
• Strong skills in community care coordination for older people
• Effective collaborator across health and aged care services
• Experience supporting students or developing clinicians
