About the Role

As the Nurse Unit Manager – Geriatric in the Home (GITH)/Integrated Care (IC) Geriatric Services, you will lead a dynamic and evolving service delivering restorative, patient-centred care to people aged over 65 across CALHN. Drawing on your consolidated clinical experience, you will provide expert nursing support and ensure high-quality intervention within a model designed to keep older adults safe, supported and well-connected across acute, sub-acute and community settings. Your leadership will help shape a service that promotes independence, reduces unnecessary hospital presentations and enhances quality of life for older clients.

In this pivotal management role, you will coordinate multidisciplinary team activities, oversee patient care pathways and maintain strong partnerships with both Government and Non-Government providers to ensure seamless, non-duplicated service delivery for clients with complex needs. You will manage the operations of an off-site location, ensuring a safe and effective environment for staff and clients, while driving positive team culture, professional accountability and consistent, high-quality care. About You

You are an experienced and forward-thinking nursing leader who brings strong clinical capability, proven management experience and a commitment to delivering safe, person-centred care. You excel in complex environments, guiding teams through change, supporting professional growth and improving models of care to enhance outcomes for older adults. Your leadership style is collaborative, inclusive and grounded in high-level communication, problem-solving and negotiation skills, enabling you to work effectively across multidisciplinary settings.

Key Requirements

  • Professional registration & clinical expertise
    Registered Nurse/Midwife with current (or eligible) practising certificate, at least three years’ post-registration experience and demonstrated competence in the relevant clinical specialty.

  • Leadership, management & change capability
    Experience leading teams, supervising staff and students, facilitating change management, fostering staff development, and promoting consumer engagement initiatives.

  • Person-centred care & service improvement
    Ability to review, enhance and innovate models of care, ensuring they are responsive to the needs of clients and families, and aligned with contemporary nursing and health care standards.

  • Regulatory, safety & sector knowledge
    Strong understanding of legislation, industrial agreements, competency standards, National Safety and Quality Health Service Standards, hospital-avoidance strategies and the complexities of caring for clients with complex physical and mental health needs.
