About the Role:

We are seeking a passionate Physiotherapist to make a meaningful difference in patients' lives through our innovative Northern Reablement and Intermediate Care to return Home (NRICH) service.

The Northern Adelaide Local Health Network (NALHN) is a leading provider of public healthcare services in South Australia.

In this role, you'll work within a dynamic multidisciplinary team, supporting our transformative approach to healthcare delivery by helping patients regain independence and confidence as they transition from hospital to home. The Northern Reablement and Intermediate Care to Return Home (NRICH) Service is delivered in partnership with the Aged Care and Housing (ACH) Group, at the HEALTHIA Residential Aged Care Facility, Elizabeth Vale. ACH Group provides specialist aged care support for NRICH patients across 24 hours, 7 days per week. The NALHN Geriatric Service multidisciplinary team provides medical oversite, care planning and a reablement program to optimise functional independence in a home-based setting to support safe discharge to home.

To be eligible for this position, you must hold an appropriate Degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia.
