    • AHP1 $71,217 - $86,938 p.a (pro rata)
    • AHP2 $91,785 - $106,330 p.a (pro rata)

Join our dedicated team delivering the Transition Care Program (TCP), a short-term, restorative service designed to support older people transitioning from hospital back into the community or residential care. HOW WILL YOU CONTRIBUTE?

  • Assess, plan, and implement occupational therapy interventions tailored to clients’ needs within the Transition Care Program, promoting independence and safe community living
  • Collaborate closely with health professionals, community service providers, clients, and their families to ensure integrated, person-centred care
  • Support and mentor less experienced staff, allied health assistants, and students (AHP2 role) or contribute under supervision (AHP1 role)
  • Assist in evaluating client progress and contribute to continuous quality improvement initiatives within the service

WHAT ELSE DO YOU NEED TO KNOW/WHAT IS ESSENTIAL FOR SUCCESS?

  • You will be required to hold registration with AHPRA to practice as an Occupational Therapist within our organisation.
