Make an Impact: Older Persons Assessment/Residential Care Outreach ANUM

As the Associate Nurse Unit Manager (ANUM) Older Persons Assessment / Residential Care Outreach Ward, you will play a critical role in ensuring seamless care transitions for elderly patients between the hospital and residential aged care facilities. This multifaceted position requires a skilled and compassionate registered nurse to conduct comprehensive assessments, coordinate admissions and discharges, and collaborate closely with interdisciplinary teams and aged care providers.

About the role: 

Reporting to the Nurse Unit Manager (NUM), this role is responsible in using their clinical knowledge and experience to provide co-ordination of patient/client care delivery within the Older Persons Assessment (OPAL) /Residential Care Outreach (RCOT) Ward. The main focus of this role is to support the ward and the NUM by providing additional management, coordination and leadership guidance for the nursing team to achieve continuity and quality of patient/client care and outcomes. Assist the Nurse/Midwife Unit Manager or equivalent in undertaking ward/unit/service management responsibilities, e.g. Performance management processes, recruitment, staffing, leave management, rostering, work allocation and attendance management, financial and supplies planning and monitoring. 

Duties for the role will include:

  • Provide proficient, person centred, clinical nursing/midwifery care and/or individual case management to patients/clients in a defined clinical area,
  • Assists and supports the Nurse/Midwife Unit Manager or equivalent in management, clinical, and education activities, support bed flow and discharges across the ward, working closely with the Nurse Consultants. 
  • Participate in clinical teaching, overseeing learning experience, and goal setting for students, new staff and staff with less experience,
  • Assists and supports the Nurse/Midwife Unit Manager or equivalent in management, clinical, and education activities,
  • Promote continuity and consistency of care in collaboration with the Nurse/Midwife Unit Manager or equivalent of the ward/unit/service

About you:

  • Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate,
  • Registered Nurse/Midwife with at least 3 years, full time equivalent, post registration experience,
  • Demonstrated competence in specialised care of the older person in nursing practice in accordance with the relevant standards,
  • Experience in the leadership and direction of student nurses, enrolled nurses and less experienced registered nurses
