About the Role:

The Northern Adelaide Local Health Network is seeking passionate and experienced Nurse Consultants to join our Northern Adelaide Geriatric Service.

The Northern Adelaide Local Health Network (NALHN) is a leading provider of public healthcare services in South Australia.

There are a number of opportunities available within Geriatrics Services, these include:

Geriatrics Outreach Service - Community Geriatrics Service, 1.0FTE ongoing
This position will be working within the Community Geriatrics (CGEM) Service providing senior Nursing assessment and review for consumers living in the community. This role will also provide support consumers who have transitioned into Residential Aged Care, working closely with facilities, consumers and their families/substitute-decision makers to prevent avoidable hospital admissions for consumers with Behavioural and Psychological Symptoms of Dementia (BPSD).
 
Geriatrics In the Home (GITH), multiple positions, ongoing and temporary
These positions will be working within our Geriatrics In the Home (GITH) Service, providing specialist senior Nursing assessment and review for consumers in the home setting. These position will also be working closely with our Older Persons' Emergency Assessment and Review (OPERA) Service where they provide support on discharge from OPERA back into the community. 
 
Inpatient Sub-Acute Geriatrics - 1.0FTE ongoing
This position will be working closely with the Geriatric Evaluation and Management (GEM) ward and Geriatric-Rehabilitation ward at Modbury Hospital, in providing senior Nursing support for consumers with complex care needs and discharge planning. You will be attending daily review and long-stay meetings, co-ordinating complex discharge planning, liaising with the home teams and other external providers including country LHN's, MRU and Home Care Providers. 

As a Nurse Consultant, you will play a crucial role in providing high-quality care and support to our geriatric patients and their families. Your responsibilities will include conducting comprehensive assessments, developing and implementing care plans and collaborating with a multidisciplinary team to ensure seamless service delivery.

Extensive experience in gerontology nursing, coupled with exceptional clinical and leadership skills, is essential. You should have a deep understanding of the unique challenges faced by the elderly population and a commitment to delivering person-centred care.

To be eligible for this position, you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
